Household Account Form Online Form - Must be completed. Please update every 2 years or when customer changes occur.  This establishes an account with which one can register!  Be sure to include an email address for online registration. (PDF version of Household Account Form)

Participant Information Form Online Form (online form being updated - not available - please use PDF version below) - Sierra Kids & Camps (Vacation Station) customers must also complete this form for each participant. This form will be kept in the Youth Office and at each location a participant attends. (PDF version of Participant Information Form)

To request a refund, a credit or a transfer please fill out this Request for Refund Form and email to lloydn@reno.gov or luop@reno.gov - FAX to 334-2449 - drop or mail to one of the addresses below.
